Frimley Health now incorporates community services, which involves integration of a range of community providers with the acute trust. We are currently looking to recruit a dynamic and innovative Community Matron to support the Aldershot and Yateley CCTs that sits within the North East Hampshire Locality.

The successful candidate must be able to demonstrate extensive post registration education within the sphere of long term conditions such as COPD, Diabetes and Heart failure amongst many others. They will need to have strong leadership skills both initiate and support change and work collaboratively within the multi- disciplinary team not just within the Frimley Health system but with key stakeholders.

You will be based in community nursing teams in either Aldershot Centre for Health or Yateley Medical Centre where your proven ability to make high-level clinical decisions will aim to prevent avoidable hospital admission for this group of patients facilitate prompt discharge from the acute setting along with supporting patients to remain independent and within their own homes/environment in the community.

You will also have responsibility for the day to day management of the other members of the community care team so that all patients within your CCT area will receive high quality timely care.
