This is an exciting opportunity to join an innovative and expanding team that has a focus on improving patient outcomes and joins up acute and community care. As part of the PACS Vanguard there has been significant commitment in the development of this service.

We are now recruiting an Occupational Therapy Team Lead within Enhanced Recovery & Support at Home. Working as part of the multi-disciplinary team you will undertake management of the service with a roughly 60% focus of your time on your own clinical caseload. The team supports patients in need of both admission avoidance and timely discharge from hospital with the priority being to keep people happy and healthy at home.

It is a varied and exciting role with ongoing development and closer professional links with the wider Occupational Therapy Teams within the Trust. You will draw on your clinical rehab skills and experience to develop patient-centred rehab goals, using professional assessment frameworks, key working and reviewing patients.

The teams consist of Occupational Therapists, Physiotherapists, Nurses and Rehab Support Workers supported by Administrators.

This is a full time position and you must be able to independently travel to and from patient’s homes and the workplace.
