We have an exciting opportunity for the right person to join our small team of Community Heart Failure Specialist Nurses within our Cardiology Department. This opportunity is open to qualified and practicing Heart Failure Practitioners.
You will be predominately community-based and will provide expert clinical knowledge and skills in the management of patients with a confirmed diagnosis of Heart Failure. You will hold a specific caseload of patients requiring some intensive support in the community, initially focusing on those patients who have unplanned admissions to hospital or those at risk of unplanned admission and newly diagnosed patients. You will work with the multi-disciplinary team to deliver a reduced hospital length of stay and also a reduction in readmission rates for patients on your caseload.
You will be expected to liaise with North East Hampshire & Farnham CCG GP Practices and relevant secondary care providers to provide assessment and follow-up of patients with a confirmed diagnosis of heart failure - ensuring optimal therapy and promotion of education towards self-management and with GPs, community services and social services to develop a multi-disciplinary approach towards improving the care and management of patients with a diagnosis of Heart Failure.
You will work on nursing issues relating to Heart Failure in line with NSF standards, local and national guidelines, influencing the Strategic planning for Heart Failure service delivery in line with NSF standards, with the support of acute and community based clinicians.
You must be able to demonstrate their commitment to continuing professional development, both on their own and within a nursing team. You will have the support of the other members of the team, as well as support from both our hospital based Cardiology Consultant lead for Heart Failure and community based GP lead.
