Fraility Unit

 

A Secondment opportunity for a registered practitioner (OT, Physio or registered nurse)

Frailty Practitioner (Band 6)

 This is an exciting opportunity to join an innovative and expanding team that has a focus on improving patient outcomes and joins up acute and community care.

 As part of the Frimley System wide plans there is significant commitment in the development of this service and we are now looking to recruit a team lead and frailty practitioners. The team supports both admission avoidance and timely discharge from hospital. The priority being to keep people happy and healthy at home.

 The teams consist of Consultant Geriatrician, Frailty Nurse Consultant, Frailty practitioners, occupational therapists, physiotherapists, nurses and support workers.

 You will support the Comprehensive Geriatric Assessment process, from ED/AMU to the Frailty unit and working within the STP in order to align Frailty strategy.

 You will be able to work in collaboration across with all health and social care providers including primary care, the integrated care teams, community nurses, mental health, social care and, patients and their carers. It is essential the candidates have advanced communication skills to communicate effectively with patients and carers / relatives in difficult situations, and have the skills to communicate with people with cognitive impairment

The service currently operates Monday-Friday -8-6 with the anticipation that this will move to a 7 day 08:00-20:00 in the future.
