Discharge Co-ordinator

 

37.5hrs a week

Secondment 12 Months

 An exciting opportunity has arisen for an experienced registered nurse to apply to join the discharge team on secondment. This will be an exciting opportunity to build on assessment and care planning skills. You will be assisting in complex discharge planning, including specialist assessments under the National Framework for NHS Continuing Healthcare and Funded nursing care. Assisting with the co-coordination of discharge plans of patients with complex health care needs and those whose discharge is delayed, across any clinical specialty at Frimley Park Hospital

 You will have excellent communication skills with a flair for problem solving and the ability to work well under pressure. You will need to have the ability to work flexibly and independently but also as part of a wide multidisciplinary team.

You must have appropriate UK professional registration.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
