Following the success of a similar role at Frimley Park Hospital, an exciting opportunity has arisen within the hospital based palliative care team at Wexham Park Hospital to support the team with assessments of end of life care patients and achieving patients preferred place of care. This innovative permanent post will require an experienced nurse with knowledge in complex discharge planning and end of life care. The successful applicant will work with the clinical nurse specialists to support patient’s wishes regarding preferred place of death.
The successful applicant will work as part of a well-established dynamic multi- disciplinary team who provide specialist advice and support to patients, their relatives and all members of the health care teams with the hospital. The service is offered to patients with both malignant and non-malignant disease, and includes Palliative Medicine Consultant, Palliative Care Pharmacist, Clinical Nurses Specialists, Palliative care Occupational Therapists and pastoral care team support
